Instructions for Your First Session

Please complete the Client Psychotherapy Intake Form and review the Cancellation Policy before your first session. All appointments are provided via telehealth. Please submit completed documents securely at least 24 hours prior to your first session.

Care Coordination Across Providers

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, or other specialist), complete the Authorization to Disclose Information Form to authorize the release of psychotherapy information.
